High triglycerides โ€” the metabolic marker that responds best to nutrition

Triglycerides are one of the most nutritionally responsive cardiovascular risk markers. Unlike LDL cholesterol, which has a complex and nuanced relationship with diet, elevated triglycerides have a clear and well-established dietary driver: refined carbohydrates, sugar, and alcohol. The nutritional approach to hypertriglyceridaemia is among the most evidence-based in metabolic nutrition, with meaningful improvements often seen within 6โ€“8 weeks of targeted dietary change.

Understanding your triglyceride number

Triglycerides are transported in the bloodstream in VLDL particles produced by the liver elevated levels indicate the liver is producing more VLDL than is being cleared, driven primarily by excess substrate from refined carbohydrates and fructose being converted to fat. Where your number falls determines both the urgency and the approach.

The triglyceride:HDL ratio often more useful than triglycerides alone: A ratio above 3.0 (using mmol/L values) is a reliable marker for insulin resistance and the presence of small dense LDL the most atherogenic lipoprotein pattern regardless of total LDL. This ratio is often the clearest indicator of metabolic cardiovascular risk in the full lipid picture.

What drives elevated triglycerides

Root causes โ€” and why the driver matters for the approach

Triglyceride elevation has several distinct drivers, and identifying which is most relevant to you shapes the nutritional approach.

Refined carbohydrates + sugar

The primary dietary driver. Fructose from added sugars, fruit juice, and high-fructose corn syrup is directly converted to triglycerides in the liver through de novo lipogenesis. Refined carbohydrates elevate triglycerides through both direct hepatic fat synthesis and through the insulin-mediated fatty acid pathway. Reducing refined carbohydrates and sugar is the most impactful single dietary change.

Alcohol

Even moderate alcohol consumption significantly elevates triglycerides. Alcohol is preferentially metabolised in the liver diverting metabolic resources toward fat synthesis and inhibiting fatty acid oxidation. For people with significantly elevated triglycerides, alcohol reduction is the highest-yield single intervention after carbohydrate reduction.

Insulin resistance

Insulin resistance drives elevated VLDL production in the liver the lipoprotein vehicle that carries triglycerides in the bloodstream. Improving insulin sensitivity through nutrition directly reduces hepatic triglyceride production. This is why elevated triglycerides are such a reliable marker for insulin resistance.

Hypothyroidism

Thyroid hormone regulates lipoprotein lipase activity the enzyme that breaks down triglycerides in circulation. Hypothyroidism impairs triglyceride clearance, contributing to elevated levels. Thyroid function should always be assessed alongside elevated triglycerides that do not respond as expected to dietary change.

Familial hypertriglyceridaemia

Genetic variants affecting VLDL production or clearance can significantly elevate triglycerides independent of diet. Nutritional management remains important and effective, but the magnitude of dietary response may differ and medication may be warranted alongside nutritional support.

Medications

Some medications elevate triglycerides as a side effect including beta-blockers, corticosteroids, certain antipsychotics, and oral oestrogen (transdermal oestrogen does not have the same hepatic effect). A full medication review is part of every triglyceride assessment.

What works โ€” and the evidence behind it

Reducing refined carbohydrates

The most impactful single dietary change more so than reducing dietary fat. Removing added sugars, refined grains, fruit juice, and ultra-processed foods significantly reduces hepatic triglyceride production. Response is often visible within 4โ€“6 weeks of consistent change.

Omega-3 fatty acids

EPA and DHA from fatty fish and quality supplements reduce triglycerides through multiple mechanisms reducing VLDL production, activating PPAR-alpha, and improving triglyceride clearance. Higher-dose EPA+DHA (in the range studied for triglyceride reduction) has strong evidence, though the right dose for you is best determined individually given potential interactions with blood thinners and other medications.

Alcohol reduction

For significantly elevated triglycerides, this is the highest-yield intervention after carbohydrate reduction. The triglyceride-lowering effect of alcohol reduction is rapid and substantial typically evident within 2โ€“3 weeks of elimination or significant reduction.

Blood sugar regulation

Low-glycaemic eating patterns, protein-first meals, and reduced refined carbohydrate intake all reduce the insulin stimulus for hepatic VLDL production addressing the insulin resistance driver of elevated triglycerides at its root.

Dietary fibre

Soluble fibre from oats, legumes, and flaxseed slows carbohydrate absorption, reduces post-prandial glucose and insulin, and feeds beneficial gut bacteria producing short-chain fatty acids that improve insulin sensitivity all relevant to triglyceride management.

Movement after meals

Physical activity directly increases triglyceride clearance through lipoprotein lipase activation. Even a 10-minute walk after meals provides measurable benefit. One of the most rapidly effective interventions for elevated triglycerides alongside dietary change.
